Provider Application and Credentialing
Wisconsin River CCS Consortium is committed to ensuring that CCS providers demonstrate competency and quality in provision of services to our consumers. CCS providers must meet Department of Health Services Comprehensive Community Services standards (DHS 36) in order to provide psychosocial support services.
Agencies or individuals may submit the Provider Application and complete the credentialing process with the Wisconsin River CCS in order to become eligible contract providers.
Counties within the Wisconsin River CCS Collaboration only contract with providers who become credentialed through the process outlined above. Each county establishes their own individual contracts with providers once they are credential. Providers must contact individual counties to learn about the contracting process.
